Identification Requirements and Parents Information:
If you are 18 YEARS OF AGE OR OLDER you must have government issued photo identification with date of birth on it and acceptable forms of identification.
If you are IF YOU ARE 18 YEARS OF AGE OR YOUNGER (minor) your parent or legal guardian must be present. They must have their government issued photo identification with date of birth on it, AND you as the minor must also have a government issued photo identification with date of birth on it and/or a birth certificate and school picture id.
